I understand and acknowledge that my stylist is to provide a chemical service to me/my child. I understand that the service has some risk of damage to the hair/scalp. Because of this risk, I/my child could sustain personal injuries, property damage, loss, expenses and/or damage to the hair/scalp. And I hereby release my stylist from any liability caused by this service. I further understand that information not provided to me/my child's service provider such as medication, previous chemical processes in a salon or at home within the year, allergies, previous issues resulting from chemical service and/or previous damage to hair/scalp will result in no guarantee of such chemical service. *

I understand that if I experience dissatisfaction for any reason with my chemical service I am to inform my stylist prior to leaving the location when possible and in any cases, I am to voice my concerns no later than two days after my visit. I further understand, due to the nature of corrective services, my stylist will not guarantee the result of colour correction. *

In the event you need to cancel or reschedule an appointment, please advise us at least 48 hours before the start of your appointment to avoid a penalty. *
